Variable Clinical Course of Lichen Planus After HCV Cure With Direct-Acting Antivirals
TAKE-HOME MESSAGE
Six patients with lichen planus (LP) in association with hepatitis C virus (HCV) infection were treated with direct acting antiviral agents. Following HCV cure, 3 (50%) patients' LP resolved, whereas 3 (50%) patients' LP worsened. Combining the outcomes in these patients with those described in the literature (n=22) demonstrated that 50% of patients' LP resolved, nearly 32% improved, and approximately 18% persisted or worsened after HCV cure. Pure oral lichen planus was more likely to resolve with cure (P < .025); of those with improvement or resolution, 66.7% had pure oral disease.
- There is a range of responses of HCV–associated LP to viral cure, from complete resolution to worsening of disease. This study may assist physicians in managing the expectations of patients with HCV–associated LP. Larger prospective trials are needed to better characterize outcomes after HCV treatment.

Lichen planus (LP) is sometimes associated with hepatitis C virus (HCV).1 No large studies examine LP outcomes after HCV treatment with Direct-Acting Antivirals (DAA). Case reports and series chiefly describe oral LP (OLP) in Japan, where OLP has high prevalence and is strongly associated with HCV2; generalizability to other populations is unclear. Prior reports are also limited by reporting bias, describing primarily extreme outcomes (complete resolution or incident disease). To overcome these deficiencies, we performed a systematic electronic record search to identify patients with LP and DAA-treated HCV at two major medical centers in the US and described the clinical course of their LP following HCV cure.
