Efficacy of Oral Zinc and Nicotinamide as Maintenance Therapy for Mild–Moderate Hidradenitis Suppurativa
TAKE-HOME MESSAGE
- Patients with mild to moderate hidradenitis suppurativa who had all achieved clinical efficacy with a 12-week course of minocycline were given either a maintenance regimen of combination zinc 90 mg and nicotinamide 30 mg daily for 12 weeks or no treatment. At 12 weeks, severity (4.0 vs 6.2; P = .004), pain (2.0 vs 6.5; P = .003), and DLQI (2.9 vs 7.5; P = .002) scores were all significantly lower among treated patients. These differences were also maintained when patients were off supplements at 24 weeks. The maintenance intervention significantly improved the disease-free interval (20.4 vs 5.4 weeks; P = .001). In the intervention group, 2 patients experienced nausea but did not discontinue treatment.
- Many patients with hidradenitis suppurativa seek out complementary therapies, including zinc and nicotinamide. The findings of this study suggest that a combination of zinc and nicotinamide may be well-tolerated and effective in improving clinical outcomes and disease-free interval in patients who have completed a course of antibiotics with clinical efficacy.

Abstract
Hidradenitis suppurativa (HS) is a chronic cutaneous disease that involves follicular occlusion in the apocrine gland-bearing regions. Treatment is a challenge because of the paucity of effective therapies and frequent exacerbations, with a negative impact on quality of life.1 Brocard et al first described zinc gluconate (90 mg daily for 4 months) as an effective therapeutic alternative for the management of HS.
