Fixed Food Eruption Caused by European Spider Crab in a Child
TAKE-HOME MESSAGE
The authors report the case of a recurrent erythematous slightly scaly patch on the cheek of a 9-year-old girl within 24 hours after each time she ingests spider crab. Prick-to-prick test with raw spider crab and patch testing with both cooked and raw spider crab on normal skin were all negative, but patch test with cooked spider crab was positive on affected skin of the cheek. Although fixed food eruption typically resolves with hyperpigmentation similar to fixed drug eruption, this case demonstrated that the diagnosis can still be considered when hyperpigmentation is absent.
- Patch testing at the affected site with food in the same preparation that triggered the reaction was sufficient for diagnosis in the setting of a consistent history, and invasive diagnostic testing is not required.
