Melanoma 10 años despues...
Mi afan no es asustar, si no llamar la atencion, de que el seguimiento de algunos pacientes con melanoma, debe ser constante, vigilante y de por vida.
- Un cohorte de 1372 pacientes con melanomas en estadío I y II, libres de enfermedad a los 10 años, luego de su diagnóstico, ocurrieron metastasis en un 6% (n = 72) de ellos. Los sitios más freceuntes fueron los ganglios regionales (n = 33), metastasis en transito (n = 21), cerebro (n = 6), y pulmon (n = 6). Los factores de riesgo luego del análisis estadístico fueron: edad antes de los 40 años, breslow > 2mm, nivel de clark IV/V.
- Las células de melanoma pueden permanecer en remisión en ganglios cerebro y pulmón, por lo que los pacientes con estos factores de riesgo, deben de ser seguidos por más de 10 años.

No creer todo lo que nos dicen en la Tele!
Study: Physician TV Personality Delivering Medical Advice Lacking Scientific Basis.
The Los Angeles Times (12/20, Kaplan) "Science Now" blog asked, "What do real-world doctors have to say about the advice dispensed on 'The Dr. Oz Show?'" According to a research published in the British Medical Journal, "less than one-third of it can be backed up by even modest medical evidence."
The Hill (12/19, Ferris) reported that Mehmet Oz, MD, the television "personality who describes himself as 'America's doctor' has been widely delivering medical advice with zero scientific basis." After investigating 80 claims made on the show in addition to 80 recommendations made on a British medical TV program, researchers found that approximately "half of recommendations from" Dr. Oz "have 'no evidence' or are flat-out contradictory to medical research."
TIME (12/20, Park) reported that the experts who conducted this study "aren't the first to cast doubt on the quality of advice given on the" show. This past "June, a Senate subcommittee heard testimony from Oz on false advertising of weight loss claims and Sen. Claire McCaskill [D-MO] queried the doctor about the statements he made on the show." The Oregonian (12/19, Terry) and the New York Post (12/20, O'Neill) also covered the story.

Melanoma 10 años luego...
Buenas, este es un estudio interesante donde pacientes que tuvieron melanomas no agresivos, pero con ciertas caracteristicas de riesgo, presentaron recurrencias, inclusive 10 años despues.

- Among a cohort of 1372 patients with stage I/II melanoma who were disease-free at 10 years after original diagnosis, metastasis subsequently occurred in 6% (n = 72). The most common sites of metastasis were regional lymph nodes (n = 33), in-transit skin metastases (n = 21), brain (n = 6), and lung (n = 6). Risk factors for late relapse on multivariate analysis were age <40 data-blogger-escaped-1.65="" data-blogger-escaped-breslow="" data-blogger-escaped-thickness="" data-blogger-escaped-years="">2 mm (HR, 2.1), and Clark's level IV/V (HR, 2.1).
- The authors conclude that physicians should monitor these patients for 10 years, paying special attention to lymph nodes, brain, and lung as common sites for relapse.
