Exposure Analysis Using X‐Ray Fluorescence Device and a Cobalt Spot Test in Four Patients With Cobalt Allergy
TAKE-HOME MESSAGE
- These authors use four case reports to describe the utility of the handheld x-ray fluorescence (XRF) device in exposure analysis in patients with cobalt contact allergy.
- The XRF device detected the presence of cobalt in various sources, and it is a promising screening tool for testing metal composition in leather products. The cobalt spot test is also helpful at elucidating sources of exposure.

Cobalt causes allergic contact dermatitis and is commonly found in metals, jewelry, leather, and tools among other things. A cobalt spot test is available. This tool is similar to the dimethylgloxime test for nickel such that a cotton tip with a reagent is rubbed on the product, and in the presence of cobalt, an indicator color is identified. This study introduces the use of a hand held x-ray fluorescence device to detect the presence of cobalt and reportedly is able to do so even if the cobalt test is negative. These tools enhance analysis of cobalt exposure in those with cobalt allergy. The authors report on this new tool that can be used for detecting the presence of cobalt. The device "bombards the material with high-energy X-ray beams…and presents the content of each element." The limited availability of this tool makes these findings interesting, but not currently far-reaching or practical. As such, we are presented with an interesting possible future tool, but are presently still left with the cobalt spot test to identify the presence of cobalt in items.
