- Trachyonychia, which may be idiopathic or associated with lichen planus, psoriasis, and alopecia areata or universalis, is a therapeutic challenge. Topical calcipotriol/ betamethasone dipropionate, topical tazarotene, oral cyclosporine, and oral acitretin have shown limited efficacy.
- The authors recommend using triamcinolone acetonide (2.5 mg/mL in 1% lidocaine 0.1–0.2 mL) injections to the nail matrix every 4 to 8 weeks as a well-tolerated and effective treatment option that improves the nail texture and functionality in this condition.

Abstract
Trachyonychia is a nail condition in which patients present with roughness and longitudinal ridging of the nail plates, often likened to sandpaper (opaque) or less commonly pearlescent nails with longitudinally arranged pits (shiny). Trachyonychia is more often seen in children and most cases are idiopathic, but it is also associated with lichen planus, psoriasis, and alopecia areata/universalis.1 Data on trachyonychia treatment in adults is limited to open label studies, case series and case reports utilizing topical calcipotriol/betamethasone dipropionate, topical tazarotene, oral cyclosporine, and oral acitretin, with limited efficacy or adverse effects. Trachyonychia is also often refractory to treatment and more likely to exhibit a chronic course in adults. Thus, a safe and effective long term intervention is necessary.
