Influenza Vaccine–Associated Cutaneous Lupus Erythematosus
TAKE-HOME MESSAGE
- A 53-year-old woman with a 19-year history of systemic lupus erythematosus (SLE) controlled on hydroxychloroquine, mycophenolic acid, and prednisolone received her annual influenza vaccine on her right posterior arm. Then, 3 days later, she developed an expanding annular erythematous plaque at the site of the vaccination. Biopsy revealed findings consistent with acute cutaneous lupus erythematosus (CLE). The lesion slowly resolved over the next 4 weeks without treatment.
- While minor cutaneous reactions to the influenza vaccine have a higher reported rate in patients with SLE than in healthy controls, the authors point out that this is the first reported case of CLE induced by the vaccine.

Abstract
A 53-year-old woman presented with a large skin lesion on her right arm. The patient had systemic lupus erythematosus (SLE), which was controlled by a stable regimen of hydroxychloroquine 200 mg twice daily, mycophenolic acid 360 mg twice daily and prednisolone 5 mg daily. The only cutaneous manifestation of her disease had been a malar rash at diagnosis 19 years previously.
